21 days is a good amount of time, if you have them for the trek. otherwise you will have to count 4 days for transportation and kathmandu, which would mean, that you have to hurry a little bit.
you just need to get the ACAP Permit in Kathmandu (it takes about 30 min/office closed on saturday) and then teke the bus the next morning to besisahar. If you dont go with an agency there is no need to fix anything in advance, as you can get normaly the tickets at your hotel, at least the ones to dumre, where another bus is going to besisahar. For the direct bus you have to go to the busstation.
if you run out of time, you can save 4 days by flying out of jomsom after crossing thorong la.
But dont hurry to much, there are many magic places on AC.
