If your fitness is a real issue you can fly into Jomsom and walk back and down... This will eliminate the uncertainty of the Jomsom to Pokhara flight the other way round.
Still, if you can put one foot in front of the other with any amount of coherence, I would not bee to worried. Time availability is an issue. Jomsom trek takes about 9 days. Just to go to Poon Hill and Ghandruk is about a 4 day trek.
In 9 days you could also consider to do ABC, though this is more strenuous.
