I go to KTM frequently. I like to stay near Durbar square so I stay at the Eden Hotel on Freak St. Close to everything, nice restaurants in the area and I pay about 300 rupees for a nice room with bath and shower in the room. Thamel is a 15 minute walk if I need to be in that area. Thamel is noisy at nite whereas Durbar area is pretty quiet except for the dogs that seem to like to bark in the middle of the nite.

KTD Guest House still has that charm that made it famous, but it is overpriced these days for what it offers. If you want to stay in Thamel, Hotel Utse or Hotel Norling are good, or perhaps Kathmandu PEace Guest House at the end of Thamel in Paknajol offers a nice quiet location. Eden Hotel in the old Freak St that Michael is talking about sound nice too.
If you can affort 100 USD per night stay at Dwarika Hotel, luxurious, and built in traditional Newari stile. It is absolutely amazing. At least you can stay there for one night to reward yourself after the trek. It is not in Thamel though.
